You need to play through a rng based bullet hell set of bosses with numerous one hit KO character multiple times to get a part of completing this thing completely

The best part of Isaac is that achievements are mostly tied to unlockables.
The 1001% was just a marker of progress
It took NorthernLion, one of the more well known players roughly 1000 episodes to get that
